My Mercy Chart

MyMercyChart—Mercy Hospital’s Patient Portal—is an online tool that allows patients to conveniently access portions of their hospital electronic medical record and view test results and other key information, 24 hours a day, 7 days a week.

MyMercyChart is free to use, simple to sign-up for and is completely secure. Only you and your Mercy healthcare team at the hospital will have access to your private health information.

For technical assistance or to reset your password, please call 1.800.324.8163. Please do not call your primary care physician.

How do I enroll?

My MercyChart is as easy as 1-2-3-4.

  1. Speak to a member of your healthcare team.

    To begin the enrollment process, provide your current email address to a member of your health care team, at your next visit.

  2. Check your email

    We will send you a message with a link to the MyMercyChart log-in screen. Click on the link.

  3. Verify your identity

    We will ask you to give us your date of birth and the last four digits of your medical record number. Please also read and accept the terms of use and privacy policy.

  4. Create your account

    Simply enter your name, email address and date of birth. Then create a user name, password and a new security question.

Then you’re done!

What is MyMercyChart?

MyMercyChart offers patients personalized and secure online access to portions of their electronic medical records that your hospital, doctors, and nurses use to manage and document your care. It enables you to securely view information about your health. With MyMercyChart, you can:

  • View hospital test results
  • Access personal medical records
  • View, download and transmit your health information

Is there a fee to use MyMercyChart?

My MyMercyChart is a free service offered to our patients.

How do I sign up?

Patients who wish to participate may request an invitation during their hospital inpatient or outpatient visit, or via a visit with their participating healthcare provider. This invitation will enable you to login and create your own user ID and password. You will be asked to provide your email address to begin this process. A valid picture ID is required to verify your identity.

How do I claim my invitation?

An invitation will be sent to the email address you provide at the time of your request, during sign up. Once you have received the email, follow the directions on the screen. The step-by-step process will allow you verify your identity by answering a challenge question and entering your date of birth. You will then be guided through the process to build your own username and password.

Your Medical Record

When can I see my test results in MyMercyChart?

Your test results will be viewable in MyMercyChart immediately after the results are put into your health record. Tests of a very sensitive nature, like pregnancy, HIV, sexually transmitted infections and pathology, are not released to MyMercyChart patient portal.

Why are certain test results not shared electronically via MyMercyChart?

Tests of a very sensitive nature are not released to MyMercyChart.

If some of my information on MyMercyChart is not correct, what should I do?

If any of your information in MyMercyChart is not correct, please contact Health Information Management at 312.567.2240. Please do not call technical assistance or your primary care physicians.

Can I enter my own personal health information in the MyMercyChart?

Yes. Under Account Settings, select "change settings with Cerner Health," and select the Health Record tab to add your personal information. This information is for your own reference and will not be part of your hospital medical record. You may share this self-entered information by selecting "Share This Record." To return to MyMercyChart, click on "Return to MyMercyChart."

My Mercy Chart For My Family

Can I view a family member's health record in MyMercyChart?

Yes, this is called proxy access. For a child under 13, this allows a parent or legal guardian to log into their personal MyMercyChart account, and then view the information regarding the child. Per State and Federal Guidelines, after the child becomes 13 access to the minor's electronic medical record will be terminated. The parent(s) will receive a warning email one day before the minor’s 13th birthday saying access will automatically be terminated at age 13. Once the child turns 13, the parent can obtain access to the patient’s medical record information with the patient’s authorization using Mercy Hospital's Authorization for Release of Health Information form. It is also possible to request access to another adult’s health record if you help manage that adult’s medical care by completing a Consent for Designation Care Manager form.

Can my spouse and I share one MyMercyChart account?

For your privacy, you and your spouse must maintain separate accounts in MyMercyChart.

Click the "Forgot password" link on the sign-in page to reset your password online. You may also call 1.800.324.8163 to request a new, secure password. Is MyMercyChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure invitation process, personal IDs and passwords. Each person controls their password, and the account cannot be accessed without that password. Furthermore, MyMercyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyMercyChart.

What is your Privacy Policy?

MyMercyChart is owned and operated by Mercy Hospital & Medical Center and Cerner Corporation. It is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by MyMercyChart.

I was logged out of MyMercyChart, what happened?

We aim to protect your privacy and security of your information. While logged into MyMercyChart, if your keyboard remains idle for 20 minutes or more, you will be automatically logged out of MyMercyChart. We recommend that you log out of your MyMercyChart session if you need to leave your computer for even a short period of time.

What do I need to use MyMercyChart?

You need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer).
